Answer: I would call and check with your lawyer or who ever is handling your case what exactly you need. They might also require a form filled out from where you did community service so without knowing your specific state and sentence it is impossible to tell.

Answer: I think you have a different issue. What are you serving and upon whom? You don't have to serve your own attorney. If you are the defendant, you do have to serve your answer upon the party who filed the complaint against you. In that case, or for any other document, a certificate of service is a separate document stating that on X date, the undersigned served (document) on (party) at (Address) via (method of delivery, i.e., regular mail, overnight mail, hand delivery).

Answer: very confused by your question. In most other states, you get a court date, judge questions both parents, makes sure step parent has had home study done and approved and Judge terminates non custodial parents rights and an appeal time is put in place and then an adoption hearing is scheduled.

They all require a POS; however, it's a general question and does not pertain to a particular state - UGG! I have been researching like mad and I would have so agree, so far, that production of documents is the best possible answer for this terrible question.
Answer: The correct answer depends on the rules for the particular court for the particular jurisdiction. It also depends on rules for service of process. A complaint is served either in hand, left at the abode of, by publication or by other means set forth in the court rules. The other documents would probably be properly taken care of by a certificate of service or mailing.

Thanks Need. Did this, but although I know what a Cert. of Service is, I couldn't find distinct wording for a Notice of Hearing on a "Motion to Dismiss" (only other hearing notices which I can reword but don't want to).
Answer: If you are familiar with the heading of a cause document, then you would follow the same format for the header, which details the cause number, the court it is being held in, the Plaintiff's information and the Defendant's information.
You then title it: NOTICE OF HEARING ON MOTION TO DISMISS
In the subsection (area below the title above), you would type: PLEASE TAKE NOTICE that [you - I imagine you are the Defendant???], will move to dismiss this action, in accordance with the accompanying Motion. Hearing is set for [Date and time] at [place - should be same court].

Type: Pro Se (as I assume you are doing this yourself)
Certificate of Service is just like any other (as you said you were familiar with them).
Attach the original Motion to it and send it certified mail, fax, etc (whatever the rules of service are in your state), but i would do all of the methods of service.
Question: Does opposing counsel receive a copy of certificate of service along with document being served?
Answer: ordinarily, yes. Theoretically they shouldn't, because the certificate says it has already been put in the mail and one shouldn't sign that until is it is actually dropped in the mail, but still, most times they get it.

Answer: Such motions are rarely granted, and doing them pro se takes the chances of success down near zero.
A certificate of service is pretty easy though. There should be a form on the website of the US District Court you're in.

Question: What do you do when someone wants to redeem a gift certificate for services no longer offered? The gift certificate was donated over 2 months ago from my personal concierge service to an organization for a silent auction. I haven't heard a peep about it until now and unfortunately assumed it was not purchased. Answer: Depending on how much was spent to win the gift certificate, you can either offer to refund her the amount she spent or hire someone else to provide the services and eat the cost. I would honor the request since the services were offered at the time and you probably don't need the bad rap.

Answer: They don't give "service certificates" in the United States at all. Your wife's work as a teacher doesn't qualify as national "service"; it qualifies as a job.

Thank you everyone for answering so far. I have done my own investigating as well. I called the department responsible for birth records in the county of my birth. I asked them if these sites are legitimate and trustworthy. He said, absolutely and suggested vitalchek.com. The reason one would go through them vs. directly through the state is that they will ensure the certificate is shipped to me in 3-5 days vs. the normal waiting period of 12 days or more. Additionally, they are certified through the vital records department and I can do my verification paperwork through them (they require a notarized form certifying identity) vs. needing to go down to the office or send in the paperwork via snail mail--which takes additional time.
Answer: Your best bet would be to get your birth certificate from your state's vital records department rather than online, especially if you need one that is certified/notarized. Some online sites may charge you two or three times what it would cost to go through your state capital. Also, there are many places that won't accept a birth certificate unless it IS from a state agency. You would have to supply the date of birth, parents names, city and county, hospital (if known), child's name, and your relationship (if it is for you, you'd put "self"). I just checked on a couple websites, and they charge anywhere from $19.95 to $24.95 and above.
