The form sent to employers from the State child support enforcement agency ordering the employer and its health care plan administrator to enroll a noncustodial parent’s child in health care coverage when such coverage is available through the employer and required as part of a child support order. When properly completed, the NMSN constitutes a Qualified Medical Child Support Order, a document necessary for health care plans to enroll dependents who are not residing with the covered parent. The NMSN is designed to simplify the work of employers and plan administrators by providing uniform documents requesting health care coverage.

Question: Where is a good place to advertise for parents who want to work from home for a company in the medical field? I noticed that many of the places are filled with these get rich quick,scam sort of companies and I prefer not to be associated with them. We have been around since 1992 and are members of the US Chamber of Commerce, National Health Alliance, National Dental Association just to name a few. We have full training and support so we do not need people with prior experience. It is mostly computer work. I already have ads on Monster.com and craigslist and I have good results. I am looking for FRESH ideas.

Question: Health insurance in USA is a complete rip off, scrap it and lets get competitive pricing for national service? I noticed my old friends in the USA are having a big debate about healthcare, getting a better service for all and at a better price. This is why I KNOW that Americans are being scammed and ripped off by the Government, Health insurance and the hospitals. I am from Uk but spent 7 years there in San Diego, Houston and Phoenix and I needed treatment 3 times for various things.
On all 3 occasions I was charged a small fortune ($2000) for menial medical work, (a stitch in my lip and check pulse, most of the time I spent waiting to be seen, (hours). When I told them I had no medical insurance they reduced it to $300. Case 2 Blood tests ($940) but if I paid cash $130, Sharps Hospital San diego. Houston local doctors remove a boil on my back $260 but for cash $45. Dentists San diego all treatment less than half price just for having a $50 family discount ticket.(not insurance). I saved thousands off that ticket.
Wife had a baby, $10,000, but we had no insurance so we paid $250, (some local tax on tobacco and booze paid the rest.
I have heard some say health care in USA is far superior to the free one we have here in uk, WELL IT FRANKLY IS NOT, you have lawsuits over there in USA that prove that healthcare in UK is just as good as USA and its almost free here. True, non urgent things may require an appointment a few weeks forward but anything urgent is dealt with quickly.
The ambulance service arrive within minutes of being called and they don't carry invoices or calculators for $2000 taxi ride to hospital!
I think you should back your president, get almost free healthcare for all, get better pricing and everyone who works pays a little extra tax just as we do here in Europe, we call it National Insurance contributions, its about 5% of your capped earnings/wages, this covers not only health care but also sickness benefits, retirement pension, dental, optical, unemployment / disability benefit, child and workers tax credits Nobody is ever excluded and there is no CoPay either, They fix you no questions asked, a very comforting thought.

Answer: Okay, that is well and good ideas. But tell me who is going to pay for it? The rich? No way, they have their pockets locked shut! The poor? No, they don't have the money, that's why we need it!...No, wait, there is another group that we can jump on...yes...the middle class, they can pay for it.
Taxes will go up on everything. And what irks me is, if a couple can't afford 1 kid, why do they heve 4-5-6+ ? There is such a thing as birth control you know!
So, now we have a hard working family with 2 kids (2 because they want to go places do things and save some money), They have a health care that they have to pay for every month, hundreds, sometimes up to $1,000 a month. They have co-pays, and have a co-pay for prescriptions, a mortgage, interest, property taxes (which part go to the welfare system already), babysitting fees if the mother works, and probably does. They don't qualify for anything free because they own a house, so any county/city benefits are not open to them.
Then the family with 5+ kids who are getting: food stamps (till the kids turn 18 or move out), title 19 medical (until the kids turn 18 or move out), free dental, free babysitting (if they decide to go get a job), rent assistance, and many other things. If you add up all of what they get and reductions, they come close to what the other family is getting. And know what? These people are also living in the same areas as the other middle class people, but they are renting a house.
But there are some who fall into the cracks, and those are the ones that concern me. But many of them could get a job if they want to. Yes it would be a chore to get it all straightened out, but is workable.
But the old and infirmed, those are the ones that we should care for. They put their time in, and now they are treated badly with insurances.

What I want to know is: if this is done, does that mean that the middle class family that has been paying ALL along for theirs, are they going to have to pay more??? Actually I am in favor of it. But not if I have to pay out even more than I am now, for you see, I am of the middle class, and I pay property taxes, insurance, co-pays, and I am okay. But I know others aren't. I am sorry for them, but I didn't get anything handed to me, I had to work hard and long to get where I am, which to some people isn't very far. Answer: A combination of Republican and libertarian. I am far to the left of you, and yet I also would support a pro-life amendment to the constitution if I were American.
But I believe that you make too big a deal of circumcision. It's really no big deal for a baby boy, many consider it to be a protection against certain cancers, and if it is a family's religion to do it, then they should. (For a Jewish family it would be very offensive to their religion to prevent them from doing that to their male child.)
I believe in universal health care paid by the public. Cigarettes and junk food could be very highly taxed to help pay for it. Drunk driving? One strike and you're out for a couple of years; two strikes and your out for ten years; three strikes and you're out for life.

I have seen children who have grown up in absolutely deadly conditions because of the selfish capitalism of our North American textile companies and banana companies installed in Central America. I have wept with a teenage girl whose father had to give her up as a small child, along with her sisters, because he was only making $30 per week on banana plantations. I know of a woman working 12 hrs per day, six days per week, in a legal textile place, and yet still doesn't make enough money to feed her children.
I have seen how thousands of other small children are living in the streets in horrendous conditions, because their parents can't afford to keep them.
No, cruel, mindless, cold-hearted capitalism has no place in an advanced society. In America and in Canada there are some restrictions on it. In Canada we call it a mixed economy. But we are allowing our companies to abuse in other countries. We should forbid them to sell in our countries unless they can prove that they are treating their workers with dignity.
If they were not in those countries, then the citizens could have the freedom to develope their own industries. But they monopolize the banana and textile industries, and with their money are able to buy the governments and do what they want.
Question: Help with Military Leave Laws!!? My husband is coming home in December for a year long tour in Iraq! I started a new job over a year ago and during my interview I made it clear that I would be wanting off for R & R and also time for when he was done with the tour! My job is now giving me trouble and not wanting to let me off. I do know that President Bush passed a new law in January 2008! I was just needing some clearity to see if I would be covered under this law! Or if there is anything that could cover me with my job. Anything would be greatful and oh so helpful! The Family and Medical Leave Act and National Defense Authorization Act for FY 2008On January 28, 2008, President Bush signed into law H.R. 4986, the National Defense Authorization Act for FY 2008 (NDAA), Pub. L. 110-181. Among other things, section 585 of the NDAA amends the Family and Medical Leave Act of 1993 (FMLA) to permit a "spouse, son, daughter, parent, or next of kin" to take up to 26 workweeks of leave to care for a "member of the Armed Forces, including a member of the National Guard or Reserves, who is undergoing medical treatment, recuperation, or therapy, is otherwise in outpatient status, or is otherwise on the temporary disability retired list, for a serious injury or illness."The provisions in the NDAA providing this leave are effective as of the date of the President's signing. The Department of Labor is working quickly to prepare more comprehensive guidance regarding rights and responsibilities under this new legislation. In the interim, WHD will require employers to act in good faith in providing leave under the new legislation. Because the NDAA amends the FMLA, FMLA-type procedures should be used as may be appropriate (for example, procedures regarding substitution of paid leave and notice). The NDAA also permits an employee to take FMLA leave for "any qualifying exigency (as the Secretary [of Labor] shall, by regulation, determine) arising out of the fact that the spouse, or a son, daughter, or parent of the employee is on active duty (or has been notified of an impending call or order to active duty) in the Armed Forces in support of a contingency operation." By its express terms, this provision of the NDAA is not effective until the Secretary of Labor issues final regulations defining "any qualifying exigency." Answer: I am a Military Reservist. You are out of luck on this one. Unless you had your employer give you written statements that they would give you time off there is nothing as of yet that dictates that they must give YOU time off for your spouses leave or R&R or return from overseas.
You could try to get your spouses unit to call them and pressure them or even the call the local paper and tell them how your employer is not supporting you. But that is about it.
There have been no new rules passed down (yet) on the new law that would dictate leave for any purpose other than injury or hospitalization.
Question: "Probable" Multiple Sclerosis (MS) Diagnosis? The doctors have diagnosed my wife with probable MS. They want to start her on beta-furon (sp?) after a week of being on prednisone. She's had a spinal tap (NOT good for my hands as she used me to hold on/squeeze for support, I tell ya!!), & we have a copy of the results.
Of course, we can't figure out what the devil we're looking at. Here's what we have:
A little over a year ago, my wife had an abnormal MRI while doing some brain functional MRI trials for $$$ ath one of the local universities. Her Dr's verified what the neurologist at the University said: The pattern seen in the MRI is the same pattern seen with MS. When they added the contrast & took the MRI, none of the lesions lit up (meaning none were active).
Fast forward to now. 4 MRIs later, & no active lesions. Since the original MRI, a couple other very small lesions have shown up in her spine & brain, but none of them show up as active. She has noticed some bluriness in her left eye when she runs since last Mar/Apr 2009 that goes away within 10mins after she runs. She had a seizure about 8 months ago, & that seizure had the Dr's thinking it WASN'T MS, because seizures are usually very rarely seen/symptoms of MS. There is also slight tingling in both hands/arms that is noticed about once a week or less, so the combo of those 2 things had the Drs put her on anti seizure meds. Bloodwork has come up negative for everything under the sun:
Lymes Disease
Lead
HIV
Viruses
other tests for diseases that can mimic MS
I'll be at her Neurology appt nxt week for the diagnosis/prognosis. the Dr's think she'll be in the 30% category of patients who will maintain the level of function they currently have & not progressively get worse.
So, is this "probable" MS legit? I've read a few places that since she is showing some very small outward signs/symptom coupled with the abnormal MRIs, that this is the actual diagnosis, NOT just the Drs saying "It's probably MS". But, I can't find anything on real sites (i.e.,National MS Society, Medical websites, etc). Does anyone REALLY know what the heck this is? I'm seeing my strong, intelligent wife trying to keep positive, but each day she is becoming more & more broken. Please, any help would be greatly appreciated.

She did get an evoke potential for the problem with her left eye & also saw the optometrist. EVP showed a problem with the right eye, not the left. The ophthalmologist said it sounded like optic neuritis, (more specifically Uhthoff phenomenon) & took a picture of the optic nerve of both eyes for comparison later. No abnormalities were observed, although he said that the patient will notice something wrong before the examination will show anything.
In addition, she was in the Army for 12yrs as a vet tech, & then worked at the local animal shelter for a large municipality for 2yrs. Plus, her Mom has progressive MS & it deteriorated enough that she could no longer function & eventually died from MS complication.
Answer: Yes, it is a legitimate diagnosis. You are looking at the fact that chances are good that your wife has MS.
If you want it in terms of medical jargon: Clinically probable MS involves-
1. relapsing/remitting symptoms with one neurological sign commonly associated with MS,
OR
2. documented single episode with partial or complete recovery, with signs on examination of multifocal white matter disease
and 3) no other explanation.
The doctors you are dealing with sound very professional and trustworthy. They are also being exceptionally thorough and following the McDonald criteria, which is the protocol all dedicated physicians use in the determination of MS.
I got the diagnosis of probable multiple sclerosis at the Cleveland Clinic, and it turned out that I have the disease. Answer: I am so familiar with this. I say, at least, you've given them the benefit of the doubt by assuming that they have intelligence to understand the basics. But alas, some people just like to play games. I also believe someone must have commented on their lack of intelligence, thus the need to explain every last minute word in an explanation which I find to be condescending but that they feel is making them appear intelligent. We know what that means, lol.
Your brain, like mine, assumes people understand simple messages. But, some, will waste time arguing about which word should come first in a sentence, when time is of the utmost.
I would suggest just slowing it down for them in a non-defensive tone (not that you do). Sometimes, in my work, I will just say I am sorry (even if not my intent or true fault) and explain it very slowly..and people, just seem to accept this better. It is a kind of second chance to get them to understand in a different way (some people seem to need this). But, in a medical setting, it pays to make sure the message is understood. And, perhaps, restating it and then asking for their clarification will be necessary in the future to avoid any confusion.
Sometimes, medical professionals are overwhelmed and it would also be easy to say after instructions, do you have any questions regarding the contact information I have given you?
